Care for Complexity in Community Health: the 3C trial

<p>A report describing a feasibility trial of care coordination for people with multimorbidity and psychosocial complexity, based on a Minimally-Disruptive Medicine approach. Trial was undertaken over 2021-2023 in a regional community health setting.
